Helios Care has received a $195,000 grant from the Mother Cabrini Health Foundation to support its Choices program, an initiative that provides in-home palliative care to older adults in Delaware, Otsego, and Schoharie counties.
The Choices program is designed to help patients with serious illnesses manage symptoms and maintain their quality of life while remaining at home. As New York’s aging population grows and healthcare access remains a challenge, such services are increasingly essential in reducing pain, isolation, and barriers to care.
The Mother Cabrini Health Foundation, a nonprofit organization dedicated to improving healthcare access for vulnerable communities, has awarded more than 500 grants in its 2024 funding cycle, totaling $208 million across New York State.
